What We Want:
Education: BA/MA/MS or BSW/LLMSW/LMSW or LLP or LPC degree in human services area
What We Dream:
One year experience with children
Coordinate person-centered planning process for assigned consumers and families.
Provide recovery-focused treatment as outlined in person-centered plan
Provide a minimum of 4 hours monthly face to face clinical contact
Coordinate and monitor community based living arrangements.
Complete CAFAS/FECFAS/DECA assessments at designated intervals.
Act as liaison to family/probate court, Juvenile Justice, schools, ISD, MDDHHS/CPS and Foster Care and Law Enforcement to assure coordination of care and promote appropriate treatment and discharge planning.
