This guide is intended for teens aging out of the foster care system or individuals exiting an institution, who are looking forward to becoming contributing members of the Cass County Community, and who need assistance in getting established. The guide should be consulted several months in advance of the discharge date.



Mentors
Having a caring mentor is your biggest guarantee of success. A mentor listens to your problems; helps locate resources and stays in touch. A mentor warns you when you're headed in the wrong direction and encourages you when times are hard. Housing
Establishing yourself begins with securing consistent, adequate shelter.

Assistance and Counseling Services:
Fair Housing Center of Southwest Michigan, toll free 1-866-637-0733. Provides assistance for persons who encounter discrimination in the sale or rental of housing.

Pokagon Band Housing Services. (Note: Most services limited to tribe members.), 269-782-8998 269.783.0443. Food commodities for any Native American. Back rent/mortgage assistance. Help with utilities through Social Services Office.

Detox  Lee Memorial Hospital, 420 High St., Dowagiac, 269.782.8681, serves people in emergency situations for 24 hours or less, followed by a referral to Woodlands for an evaluation, which is usually performed at the hospital. Medicaid or state-funded clients in need of residential services must call Kalamazoo Access & Customer Services 1.888.373.6200 for approval of residential or outpatient services. Residential services are provided at Gilmore Center in Kalamazoo, Turning Point in Grand Rapids or Shoreline in Muskegon, 1.800.981.2481, or Benton Harbor, 1.800.926.8389.

Treatment  Local treatment options include Intensive Outpatient (IOP), treatment groups, and individual counseling.

Intensive Outpatient Treatment is an alternative to residential treatment. Woodlands provides 18 sessions in six weeks, 269.476.9781.

RECREATION:
A balanced life includes exercise and healthy recreation. Several resources are listed here for some ideas where to start, but check out activities at area churches, local public schools, colleges and universities that are often low- or no-cost opportunities for sports events, museums and family activities.

Family Counseling/Mediation/Mental Health
Repairing family ties can be an important part of re-integration. It is often wise to take a fresh look at family dynamics. Counseling services facilitate healthy relationships.

In mediation, family members who are having a dispute work with a neutral mediator, who assists them in addressing the conflict in a structured, productive setting. Financial Assistance
The Cass County Department of Human Services assists people without financial resources. Contact Sheila Beaty at 269-445-0283 and discuss your eligibility for food stamps and other forms of assistance. Most other agencies
require a letter of denial from DHS before they can assist you.

Identification and Documentation
All agencies ask for identification, but it's tough to get identification without identification. Ask for a photo ID before being discharged.

To get a driver's license you must show the examiner three pieces of identification. The ID can come from your institution. For more information contact the Secretary of State, M 51 N and Prairie Ronde St., 269-782-6512.
The cost is $25.

To get a birth certificate, you must contact the county clerk in the county in which you were born. The Cass County Clerk is located in the County Building, 120 N. Broadway, Room 123, Cassopolis, 269-445-4464. Cost is $13. You must show picture ID or three other forms of ID.

To get a replacement social security card, you must mail or present in person original documents that prove your identity. There is no fee to replace a Social Security card. The nearest Social Security offices are at:

performed at the hospital. Medicaid or state-funded clients in need of residential services must call Shoreline Consulting for approval of residential or outpatient services. Residential services are provided at
Gilmore Center in Kalamazoo, Turning Point in Grand Rapids or Shoreline in Muskegon, 1-800-981-2481, or Benton Harbor, 1-800-926-8389.

Treatment - Local treatment options include Intensive Outpatient (IOP), treatment groups, and individual counseling.
Intensive Outpatient Treatment is an alternative to residential treatment. Woodlands provides 18 sessions in six weeks, 269-476-9781.
